Making Appointments

To make an appointment please call our office during office hours. Please be prepared to give the patient’s name, the reason for the visit, referring physician if there is one, insurance information, and a daytime number. Appointments for any of our locations can be made at any of our offices in person or by phone.

  • Emergencies and acutely ill patients are seen as necessary during office hours. However, in the event of an emergency after hours, there is always a physician “on-call” 24 hours a day, seven days a week, 365 days a year.
  • Non-emergency issues will be handled during regular office hours including prescription refills and requests for medical records.
  • Please make sure you have all the prescriptions you need called in before 4:00pm Monday through Friday. Our physicians WILL NOT call in prescription refills for antibiotics or pain medications without review of your medical record.

New patients should bring all medical records pertinent to the condition for which they are coming to see the doctor. Also, please bring all insurance cards, both medical and lab. Referrals will be needed from your primary care physician if your insurance requires it. Please make sure that we have the referral documentation before your visit or bring it with you.

If you’re a new patient, you must download and complete our “New Patient Form Package” and arrive 30 minutes ahead of your scheduled appointment to give our office staff ample time to prepare your file for your appointment.
